The relationship between blood flow velocities in retrobulbar vessels and glaucomatous damage an intraindividual comparison

Purpose: To investigate, if a difference in visual field and Heidelberg retinal tomography (HRT) between two eyes of an individual glaucoma patient is reflected in different blood flow velocities in retrobulbar vessels.
Methods: Data of 53 patients: 26 normal tension glaucomas (NTG) and 27 high tension glaucomas (HTG) were evaluated retrospectively. 38 patients demonstrated asymmetric glaucomatous damage (AGD), shown by both structural (HRT) and functional measures (static or Goldmann perimetry). In 15 patients there was a symmetric glaucomatous damage (SGD). To discriminate between AGD and SGD we used a score for HRT and visual field. Color Doppler imaging (CDI) was performed with a Siemens Quantum 2000 to measure peak systolic, enddiastolic flow velocity (PSV, EDV) and resistivity index (RI) in ophthalmic (OA), central retinal (CRA) and short and long posterior ciliary arteries (SPCA & LPCA). Statistic analysis was based on intraindividual comparison (paired Wilcoxon test) of CDI assesments for patients with AGD and SGD in HRT and visual field. Groups were compared by two sample Wilcoxon test.
Results: Mean intraindividual deviation (MID) of PSV in CRA was 0,43 ± 1,59 cm/s (AGD) vs. 0,28 ± 1,00 cm/s (SGD), p: 0,87. MID of EDV in CRA was 0,12 ± 0,83 cm/s (AGD) vs. 0,44 ± 0,75 cm/s (SGD), p: 0,31. RI in CRA: 0,00 ± 0,08 vs. -0,04 ± 0,07, p: 0,30. EDV in SPCA: 0,26 ± 1,29 cm/s vs. -0,07 ± 1,08 cm/s, p: 0,41. Similar tendencies were observed for the other parameters.
Conclusions: Comparing group means of intraindividual deviation of CDI in patients with AGD vs. SGD we did not find significant differences. However, overall analysis of CDI values in NTG vs. HTG eyes showed decreased blood flow velocities for NTG eyes in SPCA, PSV and EDV.
